Name: print the Clubber's
full name large and neat so that it can be easily
read. Print it on both sides of the record.
Top
Leader: print the name of
the Leader whose group the Clubber belongs to, in
the top, right corner of the record - on both
sides. If a
Clubber changes Leaders, mark a single line
through the old Leader’s name and write the new
Leader’s name above it. Write
a date beside the new name in case we need to ask
the previous Leader about a record entry from
previous dates on the record.

Address/Phone/City/State/Zip: print the
Clubber's address, phone number, city, state and
zip code. You may not get this the first night,
but try to fill it in over time. Check your
records each week until you have all the necessary
information on your Clubber.

Bday/Age: print their
birthday in day/month/year format and print their
age beside it for quick reference.

Parents: print the
complete name of both mother and father or
guardians.

Grade: print what grade in
school the Clubber is in.

Phone Number:
try to get the phone number from the parents
if possible. Many children will know it, but some
will not.

E-mail: not necessary or
required.

Church: what church the
child attends.

Brought By: what
child or person brought the Clubber to Awanas the
first time.

Start Zone:
The Start Zone pamphlet has 7 "Checkpoints"
in it. Place the month/day in each of the 7 boxes
that each Checkpoint was passed. For example, if
Checkpoint 1 was passed on September 4th, then
place a 9/4 in the first box.

Section: Each handbook
is divided into 8 "Discoveries" or "Challenges"
(Books 1-2 have 8 Discoveries and Books 3-4 have 8
Challenges each) corresponding to the 8 rows.
Each Discovery or Challenge has 7 sections
corresponding to the 7 columns across the top. As
each section is completed, place the date that it
was completed in the box. For example, if the
Clubber completes Discovery 1, Section 3 on
September 4th, then place a 9/4 in the box in the
first row, under the 3. This would equate to row
1, column 3 which corresponds to Discovery 1,
Section 3. Follow the same procedure for all
sections.

Silver/Gold: At the
end of each Discover or Challenge, there are two
silver bonus sections and two gold bonus
sections. They are optional. The Clubber should
be given the same number of points (10) for
completing these bonus sections as a normal
section.

Club/SS Attend.:
place an X in the top box for the week that
the Clubber is attending the Awana Club and an X
in the bottom box if the Clubber attended Sunday
School, Bible Study, etc. at their Church this
past Sunday of the same week. For example, it is
the 2nd Wednesday night of September and Johnny is
present at the meeting. You place an X in the 2nd
box on top. You ask Johnny if he attended Sunday
School this past Sunday. If he says yes, you
place an X in the 2nd box on the bottom.

Attendance Awards:
this area will be filled in by the Group
Secretary. They will enter the date that the
award was given to the Clubber. Four awards are
given throughout the year. There are certain
restrictions on the number of meetings a Clubber
may miss without an excused absence. The Group
Secretary will be familiar with these rules and
they are found in the FBT Awana Leader Guidebook.

SS Attendance:
this area will be filled out by the Group
Secretary. They will enter the date that the
award was given to the Clubber. Two awards are
given throughout the year. The Group Secretary
will be familiar with these rules and they are
found in the FBT Awana Leader Guidebook.

First Book Award:
this area will be filled out by the Group
Secretary. They will enter the date that the
Clubber received an award for completing their
first book.

Excellence Award:
this area will be filled out by the Group
Secretary. They will enter the date that the
Clubber received an award for completing their 2nd
book - called the Excellence award. The Third
Book Award (side 2 of record) is for the date when
the award was given for finishing three books and
the Timothy Award is for completing four books
(side 2 of record).
